Physician Driven Disease Management

 

Disease Management Comes to Small Employers

 

Since inception, Partners In Care and the Law Firm of Hoagland, Longo, Moran, Dunst, and Doukas (HLMDD) have enjoyed a productive and effective collaborative business relationship. 

HLMDD's health care department has represented clients in nearly every aspect of health law. The Firm’s health care clients include: acute-care hospitals, physicians, multi-specialty physician practices, managed care entities, PHO/MSO's, long-term care facilities, skilled nursing facilities, pharmacies, temporary nursing agencies, emergency medical services (EMS) agencies, EMS providers (paramedics, EMTs and nurses), and all manner of healthcare employers.

In the Fall of 2003, HLMDD made the decision to self insure their health benefits for the approximately 100 participating employees at the time.  HLMDD and PIC leadership met and literally created what is today known as PIC's Health Plan Improvement Program through a series of meetings in November 2003.

This pilot has been operating since December 1, 2003 and has been more successful than anyone imagined for both parties. 

The decision to self fund their health benefits came as a result of an astronomical projected increase in their health insurance premiums.  Not unlike similar organizations their size in Central New Jersey, HLMDD found that even the Maximum Claims Projection under their self funded plan was less than the premiums they were scheduled to pay under the fully insured option ... significantly less.

As they realized the flexibility in benefit structure that Self Funding brings, they also began to realize some of the additional issues that they would face. 

They wanted an independent yet HIPAA compliant storage of their data to provide them with the freedom to manage their plan and review reports that they wanted not that were constructed by a Third Party Administrator.  PIC built one for them. 

They learned that that they could set up contracts directly with providers who were not in their existing network locally through Partners In Care and that these contracts actually saved them money, not the other way around.  PIC built this program for them.

They wanted their employees to have less hassle and more support when they were sick, and they helped PIC construct our Healthcare Advocacy service. 

They wanted a partner to work with them on assisting their employees to remain healthier and to help them shape and target their employer initiatives, PIC has been that partner, first with our Health Risk Assessments, and then with the Data Driven, Physician Focused, Care Improvement Initiatives for focused at risk members.

And ... yes ... they wanted to spend less on healthcare benefits but receive better healthcare services.  PIC has assisted in helping them stay under projection during the last two years ... significantly under projection.

During nearly two years of operation, we have helped HLMDD accomplish their goals.  With their recent addition of Egan, Amato, and O'Connor as their Health Benefits Advisors, HLMDD is poised to set the standard for the next generation in Self Funded Employee Health Benefits in Central New Jersey.
