Disease Management Comes to Small Employers

 

Partners In Care and MONOC are piloting PIC's Health Plan Improvement Program for Employers. 

MONOC, The Monmouth Ocean Hospital Service Corporation, is a non-profit company comprised of nineteen acute care hospitals located in Monmouth, Ocean, Atlantic, Bergen, Cape May, Hudson, Essex and Union Counties, New Jersey. Together this shared services consortium acts as a health care co-operative for these acute care hospitals and the over 4.3 million residents that they serve living in more than 2,500 square miles along New Jersey's northern, central and southern shore.  It includes hospitals from four of New Jersey's premier integrated health care delivery networks; Meridian Health System, Robert Wood Johnson Health Network, the Saint Barnabas Health System and the Atlantic Health Care System .

MONOC was formed in 1978 as "a cooperative venture to improve health care and reduce costs." Among its numerous projects, MONOC operates four main programs: Mobile Intensive Care Services, Mobile Critical Care Services, EMS and Transport Basic Life Support Services and EMS educational courses. Through its affiliate, the Monmouth Ocean Healthcare Co-operative, other specialty programs are provided, including group purchasing and personnel agency services.

MONOC employs over 850 staff and operates within a $67 Million annual budget. Last year, the company responded to more than 100,000 requests for emergency and medical transport services.

The PIC/MONOC Health Plan Improvement Program is utilizing the claims and eligibility data from the MONOC Self Funded Health Plan to identify the key members who need Disease, Care, and Case Management Services and that MONOC's Health Benefit Programs are optimized and aligned with their Human Resources goals and objectives.  PIC staff is collaborating with MONOC to ensure that, to the extent possible, MONOC's health benefit dollars are invested properly and directly into the delivery system.  This Program has been 2 years in planning and over eight months in implementation.  PIC and MONOC are both optimistic that benefits from this intensive effort on both sides will begin to show during the 2006 plan year.
