Physician Driven Disease Management
Disease Management Comes to Small Employers

At its June 7th, 2006 meeting, the Partners In Care, Corp. Board of Directors voted to endorse and actively drive the implementation of the Bridges To Excellence Program in Central New Jersey. The PIC Board approved an initial pilot of $30,000 in loans to its member physicians to acquire Bridges To Excellence Certification in the areas of Diabetes, Heart/Stroke, and Physician Connections. The loan program is simple. Loans will be made to practices to pursue certification. Loan proceeds can be used for application fees, for practice staff time in preparation for the certification, or to compensate PIC staff to support a practice in applying for certification. Loans will accrue interest at a market reasonable rate tied to the Prime Lending rate at the time of the loan. Loan repayment will not need to commence until BTE Program rewards are paid. Once rewards are paid, PIC will ask that 20% of reward proceeds be paid to PIC to cover the balance due, including interest until the loan is repaid. Of course, a practice can also choose to repay the loan directly prior to rewards being paid. Practices will be welcomed to avail themselves of any group purchasing discounts for application fees that PIC is able to arrange regardless of whether they seek a loan. The initial goal of this pilot is to achieve a target of 60 physicians with at least one BTE certification by June 30th, 2007. The overall goal of the Physician Driven Bridges to Excellence program is to achieve a minimum of 1,000 physicians with all three Bridges to Excellence certifications and a minimum of 250,000 patients with Employers, Union Funds, and Health Plans participating in the Bridges to Excellence program in Central New Jersey by December 31st, 2009. Achievement of this goal would be transformational for the health status of Central New Jersey residents. Using Bridges to Excellence metrics, it would save Central New Jersey more than $60 million per year in avoidable health care costs and may result in annual rewards for physicians of $8 - $12 million.
