Physician Driven Disease Management

Disease Management Comes to Small Employers

PartnersSelect: Targeted Physician Recruitment

Partners in Care has developed a network of affiliated Physician Organizations in Central New Jersey.  Common among these groups is the interest in developing “Pay for Performance” models for Physician Compensation.  The performance is measured from data collected by health plans during their normal course of business.  The Physician Organizations participate with the plans either through the present and existing networks or through a direct contract between the employer and Partners in Care.

This network is an “opt-in” network developed on an employer specific basis where, in return for the presence of “Performance Bonuses”, participating physicians have agreed to invest considerable and valuable administrative time in coordination of care and systemic quality management activities which the current health care financing system does not fund.

Numerous experts including, but not limited to, The Institute of Medicine have identified these two items as the most compelling opportunities to save both lives and money in the current system.

Partners in Care has had 8 years of experience in special performance related arrangements between Health Plans (both fully and self insured models) and Providers (including all types of providers such as Physicians, Hospitals, Home Care, DME, etc.).
