Physician Driven Disease Management

 

Disease Management Comes to Small Employers

 

Partners In Care recently met with the program leadership at Horizon BCBSNJ and agreed to expand the current pilot program for a subset of members of the New Jersey State health Benefit Program who are dealing with a chronic or complex illness.  The successes to date are helping to develop the game plan for future initiatives within PIC's programs.  PIC anticipates patient population growing significantly from the current 1,400 lives.

 

·    The first year of the PIC/Horizon SHBP Pilot has been an unprecedented success:

  • Compliance with national guidelines for the treatment of chronic and complex patients has improved substantially

  • Population costs have been reduced inclusive of pilot expenses

  • Physician income has been materially and positively impacted by appropriate and targeted investment in Value Based Healthcare Services

  • The Patient Centered Medical Home/Care Coordination Model is a better delivery model

·    The PIC / Horizon collaboration has proven that:

  • Physicians and Healthplans can collaborate to improve quality, reduce cost, and develop alternative funding models

  • Physician Organizations (IPAs, MSOs, etc.) can be complementary/non-redundant partners with Health Plans

  • Healthplan / Physician communication can be strengthened through better integration 

  • Detailed Administrative, Clinical, and Financial data can be used effectively to target programs for patients.

  • Physician Organizations can jointly collaborate with Health Plans, Pharmaceutical organizations (Merck POMS), key Technology Vendors, and Ancillary Providers to accelerate the deployment of necessary resources with reduced friction

  • A six sigma, data driven focus on total population management can be successful, scalable, and transportable

·    As a result of this success, the Board of Partners In Care has directed the following:

  • The Patient Centered Medical Home/Care Coordination Model is now PIC’s standard business practice and has been implemented across all clients

  • PIC now has 17,454 patients in 1,925 Medical Homes in 23 States on 7 platforms.  Among the 5 major plans, only Horizon and Aetna have demonstrated substantial and proactive collaboration.

 

·    Commencing in 2008, PIC is deploying an aggressive campaign designed to promote and project Physician Guided Care Coordination with the Patient Centered Medical Home as a means to transformationally improve quality and reduce healthcare costs using the foundation of Six Sigma

·    With the encouragement of some national health care organizations, PIC has launched the Partners Consulting Group to market PIC’s Collaboration-based solutions nationwide and to further broaden revenue generating opportunities for its affiliated practicing physicians
