Physician Driven Disease Management

 

Disease Management Comes to Small Employers

 

PIC packages administrative services for employers, funds, and broker/consultants to improve a pre-existing health plan infrastructure.  PIC serves as the advocate for the employer, fund, or in the case of a broker, as an extension of the broker’s services.

Having collected data and clinical notes from the Administrator, Utilization Review, Case Management, and Disease Management vendors, PIC integrates the information, validates the efficacy of the activities and provides the employer with analyses and advice on opportunities for improving the healthcare received by their employees and dependents. We work with the employer’s existing administrator and provider network.  Through this mechanism, we seek a means of reestablishing the relationship between the local employers and the local providers. 

As part of this service, PIC can develop a customized performance-based incentive plan that identifies the handful of key providers influencing the majority of care decisions for the employer’s plan beneficiaries, and rewards those demonstrating the care coordination and quality activities the employer values.  The majority of PIC’s revenue comes from savings generated by it’s activities.

Examples of areas of significant opportunity for quality improvement and cost savings include but are not limited to:

  • Asthma Disease Management

  • Specialty Pharmacy Acquisition and Delivery Management

  • Cancer Care Treatment and Prevention

  • Preventive Diagnostic Tests

  • Adverse Event Tracking, Management, and Avoidance

  • Ethical End of Life Pathways

These services are priced in three ways depending upon the desires of the client:

  • Flat Per Employee Per Month (PEPM) or Per Member Per Month (PMPM)

  • A Performance Based Shared Savings approach

  • (New) A Performance Based Shared Funding approach which integrates with your Stop Loss Reinsurance to reduce your employer's financial exposure
